you really gotta figure out and stop making things so over exposed and contrasted.

If you are applying any efx at all, stop. thats probably problem #1.
 
Personally from you, I'd rather see your next few pieces have not enough lighting on the face, because its a lot easier to learn to add a little lighting than to take away lighting.
 
So whether its efx, or a gmap, or a contrast layer, temporarily stop so we can help you identify what it is

I do use topaz sometimes and most of the time I use high pass on almost everything

From what I can tell, your radius on high pass is always too high

the highpass could be it. that along with using too much exposure takes away almost all quality.

/\ Yeah this is what it seems to be from looking at the PSD. I had to a trim a lot of those layers to redeem some of the quality. Best advice I could give in that department is try not to get into the mode of using the same techniques over again without thinking about them. Try to think about each pieces individual needs and THEN decide what effects/techniques you're going to use for it. If you end up doing a lot of the same things in your work that's fine, just make sure each piece calls for it.
